• Please file the below Motion, Entry, and Certification for Appointed Counsel Fees Form along with your appointment entry, client’s financial disclosure, and the dispositional entry with the Court within the specified timelines to receive reimbursement. The current rate per hour for appointed counsel is $75.00 in-court and out-of-court.

• If you are over the maximum fee limit for your case/offense type a motion and entry for extraordinary fees is required.

Guardian Ad Litem Application:

• Please complete this application and attach the following: background check dated within the last year, preservice certificate, and any training certificates for the preceding year relevant to performing the duties of a Guardian Ad Litem. Additionally, please read Rule 48 of the Ohio Rules of Superintendence and Madison County Juvenile Court Local Rules 25 and 26 to ensure all qualifications are met.
