Birth & Death Certificates

The Office of Vital Statistics issues certificates for all Ohio births and for deaths that occurred in Madison County from 1908 to the present. We can supply forms to add a Father's name to the birth record and make typographical corrections to existing birth or death records.

We can provide same-day service for walk in clients and take cash, checks or debit/credit cards. We can process orders that are mailed in or take orders over the phone with payment by a debit/credit card. We also offer expedited shipping by USPS for $27.00.



To order a record online or by phone:

Request a record from the convenience of your computer, tablet, or mobile device!  Just visit certificates.madisonph.org to get started.  You can also call the office during business hours: 8:30 am – 4:30 pm Monday through Friday. Be prepared to give us the information that is on certificate application and your debit/credit card number. You may choose to have the certificate delivered by regular mail or choose expedited shipping for $27.00.

  • Local fee: $11.50
  • State fee: $9.00
  • State child abuse prevention fund: $3.00
  • State domestic violence shelter fund: $1.50
